Congratulations on taking the first steps to becoming Smokefree! By stopping smoking you are doing one of the best things for your health. Your stop smoking advisor will have informed you if you are eligible to join our financial incentive scheme and this leaflet explains the scheme in more detail. For further information please speak to your advisor.

If you decide to join the scheme you will receive Love to Shop shopping vouchers at various stages throughout your quit attempt:

• One £50 shopping voucher when you have stopped smoking for 4 weeks (this must be CO verified)
• One £20 shopping voucher when you have stopped smoking for 8 weeks (this must be CO verified)
• One £20 shopping voucher when you have stopped smoking for 12 weeks (this must be CO verified)

These vouchers will be sent to you via an email

In order to be eligible for the shopping vouchers you agree to:

 Stay smokefree

• Attend support sessions throughout your quit attempt with a stop smoking advisor
• To attend appointments at week 4, 8 and 12 of your quit attempt to provide a Carbon Monoxide reading to confirm that you are smokefree

• Throughout the scheme we will ask you to complete some evaluation questions, this will help us to ensure we are providing a high quality service. Your information will remain confidential, however, if you would like more information please read ‘Confidentiality of your information’ leaftet.
